The story is that you are a child trapped in some underground human experiment lab. Something went wrong, all the subjects went savage. You, however, got very strong and kept your sanity.
Though you are not powerless, the situation you are in is horrible and that is something I try to convey through the mechanics as well. The enemies you are hitting were people you knew and even though you might be able to take them on in combat, would you? That's what the insanity meter is for. in a situation like this, fighting your way out realistically does not come without consequence and the meter is trying to mimic that.
In between punching doors down or fighting enemies in your path, you can find a teddy bear. This items helps you to keep your sanity, but that's not all. Aesthetically it is a reminder to the player that you are still a child who finds joy in innocent toys like that.
The game also touches the subject of morals a bit. Is it okay to harm people you know even if it is out of self defense?
